ILLUMINATION FOUNDATION Referral Coordinator in Orange, California

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

 

Job Description

The Referral Coordinator is responsible for evaluating and reviewing referrals of homeless clients from hospitals or other agencies in multiple surrounding counties contracted with Illumination Foundation Recuperative Care Center to ensure potential clients meet admission criteria for the program. Coordinates with hospital/IPA/MCO contact by managing the discharge of homeless client to assist in the successful transition back into the community and performs admin duties that abides by Recuperative care contract verbiage. 

The pay range for this position is $20.00 - $22.00 per hour, depending on experience. Schedule for this position is Friday - Tuesday, 1:00pm - 9:30pm.

 

Responsibilities

  • Reviewing incoming hospital/MCO/IPA client referrals

  • Gathering preliminary referral information

  • Creating referral logs, monthly summary, and data reports

  • Providing referral forms to bill the hospitals

  • Following up with hospitals for referrals and scheduling an in-service

    Preferred Experience/ Minimum Qualifications

    Required:

  • High School Diploma or equivalent.

  • Excellent organization and time management skills.

  • 1 year experience in a medical setting.

  • Customer and client management.

  • Strong self-management skills.

  • Strong computer skills including the ability to prepare correspondence and reports using Microsoft Office Suite Programs.

  • Display full understanding of the problems of client flow and scheduling, in order to improve the efficiency of program operations through performance of duties.

    Preferred:

  • Current and non-restricted Licensed Vocational Nursing.
